Actually, there is a threesome in DA:O, and there is a foursome as well.

They all involve Zevran, the bi-sexual Elf Assassin. If you're in a romance with him and take him to the Pearl in Denerim, you can have a threesome with Isabela.

The foursome happens if Leliana has been exposed and changed to a "hardened" personality, and is also there with you. The timing of this is extremely difficult, but it CAN be done.
