Download Widescreen fix by nemesis2000.
Extract mgs.7z files to the installation folder (password: mgs).
Run upx.bat if the executable size equals 1,008,640 bytes.
Edit mgs1w.ini to set the resolution.
For GOG.com version:
Download the widescreen fix by nemesis2000.
Inside the game folder, create a new folder called scripts. Extract mgs1w.dll and mgs1w.ini to the scripts folder and rename mgs1w.dll to mgs1w.asi, and edit the mgs1w.ini file to your the resolution two times lower than your actual one (e.g. 1280x720 for 1080p) or your desktop resolution (higher resolutions will cause severe lag when fullscreen effects are being applied, i.e during menus or while underwater).
Download the 32-bit version of Ultimate ASI Loader by ThirteenAG.
Extract dinput8.dll to the installation directory and rename dinput8.dll to winmm.dll
Delete mgsvideo.cfg and mgsvideo1.cfg from the installation directory.

Download dgVoodoo 2 and extract it to a different location from the game.

Copy D3DImm.dll and DDraw.dll from the MS\x86 folder to the game installation directory.
Launch dgVoodooCpl.exe as an administrator, click the add button, and select the game installation directory to create a profile.
Select the DirectX tab, force your desired resolution, uncheck dgVoodoo Watermark, and adjust vSync/MSAA/texture filtering as you wish.
For similar GOG's version graphical settings, use dxcfg.exe included with its release, the .exe is within the game's director
